Insurance
Payment in full is due at the time services are rendered.
Upon request
a Superbill will be provided. A Superbill is a receipt that you submit
directly to your insurance company requesting reimbursement for payments made. Please contact your insurance company to inquire if acupuncture is covered under your policy.
Confidentiality
Your privacy is important, and your files and patient information will be kept confidential. They will be shared only when necessary to provide care and services,
or when required by law. This office is compliant with HIPAA .
Missed Appointment Policy
If you need to change or cancel your appointment kindly do so with 24 hours
notice, allowing others to fill the spot. Otherwise you will be charged the full fee for
your appointment.
